    Dear (Name),

I am writing this request that you take this letter as a formal resignation from my position as a public relations manager with an effect from 1st March 2012.

After my maternity leave, I developed some major complications that put me under so much stress which has constantly led to depression. I am currently on anti-depressant and also going through some counseling sessions to rectify the situation.

It is with deep regret that I wish to inform you that the condition is quite serious and am therefore unable to competently carry out the duties assigned to me.

Kindly let me know if there is anything I can do to facilitate a smooth transition. Looking forward to hearing from you.

    Yours Sincerely,
